Community Action Team

 

Our Community Action Team (CAT) is a comprehensive, crisis-level program designed to support children and adolescents ages 4-21 at risk of displacement from their homes due to a variety of factors. The program ensures the well-being of youth and family through intensive, strength-based services.

This is a six-to-nine-month program consisting of a therapist, service coordinator (targeted case manager/mentor), nurse, and psychiatric APRN/psychiatrist. The team uses a strength-based perspective to assist not just the youth but the family as a whole.

Who is the Program for?

Ages 11-21

If the following applies to your child, they might be a good fit for the CAT program: 

  • At risk for out-of-home mental health or Juvenile Justice placement
  • Unsuccessful in less intensive services such as outpatient therapy
  • Baker Acted two or more times in the last year
  • Currently or previously involved with Juvenile Justice or law enforcement
  • Poor academic performance and/or have been suspended

Ages 10 and under (Little CAT)

Our Little CAT program is designed for younger children who:

  • Are struggling with challenging behaviors, such as intense separation anxiety, sleeping problems, or tantrums
  • Have issues with physical aggression
  • Have been asked to leave a daycare or preschool program

The overall goal of Little CAT is to set the family on the right path for their child’s success.

  • Decrease out-of-home placements
  • Improve family and youth functioning
  • Decrease substance use and abuse
  • Decrease psychiatric hospitalizations
  • Increase health and wellness
  • Transition into age-appropriate services (for adult clients)
